Prosedur Pendaftaran :

    • Mengirimkan email kepada PIC dengan subjek “Permintaan Form Pendaftaran Cabang Futsal” atau meminta langsung kepada PIC berupa hardcopy ( Binusian/ Employee/ Outsource ID )
    • Mengisi Form Pendaftaran dengan lengkap, sesuai dengan informasi di dalam Form tersebut.
    • Mengembalikan Form Pendataran baik softcopy atau hardcopy kepada PIC.

Requirements for Registration :

    • Send email to the PIC with subject “Request Form Registration for Futsal” or ask hard copy to the PIC directly  ( Binusian/ Employee/ Outsource ID )
    • Fill out the registration form completely, according to the information given in the form
    • Returns registration form (Soft copy or hard copy) to the PIC
